About Health Partners Free Clinic

They offer 2 weekly, walk-in clinics held on Monday mornings and Thursday evenings, registration begin 30 minutes prior to the Clinics. Insurance: Medicaid, Medicare and Insurance Eligibility: Resident of Miami County, Ohio Without insurance and/or unable to afford insurance Documents you must bring: A valid form of identification (drivers license, state ID card, etc.) Proof of financial information (tax returns, pay stubs, etc.) Services are FREE.

Mission Statement: Established in the late 1990’s, Health Partners Free Clinic operates in Miami County with the mission to provide health care for the county’s residents, regardless of age, gender, religion or ethnicity.
