To apply patients need the following: Be uninsured or lack prescription drug insurance (Medicare, Medicaid, etc.) A social security number Proof of income (tax returns, pay stubs, unemployment, spousal support, etc.) Services: Adult & Geriatric Care Primary Care for Chronic Illnesses Blood Pressure Management Diabetes Management and Prevention Program Asthma Treatment COPD Treatment Physical Exams Breast Exams, Mammogram Referrals Interpretation of Diagnostic Tests Annual Influenza Inoculation Nutritional Assessment Visual Acuity Screening Psychological Screening Mental Health Counselling and Referrals This is a free clinic. Patients are seen by appointment only; no walk-ins are accepted. Uninsured and underserved are eligible for this service.
